Chicago Booth alumni, current students, and faculty members, as well as nonprofit and government leaders, are invited to share insights about candidates applying to Booth's Civic Scholars Program. By filling out this form, you will help us learn more about promising candidates for this highly selective program designed for professionals in the public and social sectors. The information you provide will be included within the candidate's application for review by Chicago Booth's MBA Admissions Committee, should they apply. All candidates that are recommended to us through this form will have their $250 application fee waived.
